Intake Questionnaire

Typically, a complaint is initiated by filing an “intake questionnaire.”

Below are two electronic versions of an intake questionnaire that may be used to initiate a complaint. The Commission can also provide a paper copy of the intake questionnaire by regular mail upon request.

The Commission also regularly accepts complaints by alternate means to accommodate people with disabilities, address language barriers, and provide access to those who do not have Internet access or computer technology. If an individual is unable to submit a written complaint in the prescribed form, they may contact an intake consultant to find an appropriate way to provide the information necessary to initiate a complaint.

1. Intake Questionnaire*

The Intake Questionnaire form can be downloaded, filled in electronically, and in most cases, it can be submitted directly by using the “Submit Via E-mail” button at the end of the document. This form works with Adobe Reader or compatible technology. There are required fields in this form. If all of the required fields are not completed this form cannot be sent by e-mail.

2. Accessible Intake Questionnaire*

An accessible Intake Questionnaire can be downloaded, filled in electronically, and in most cases, it can be submitted by using the “Submit Via E-mail” button at the end of the document. This form works with Adobe Reader or compatible technology. There are required fields in this form. If all of the required fields are not completed this form cannot be sent by e-mail.
